HUD has posted a new Race and Ethnic Data Reporting Form HUD-27061.  This form must be maintained by the Owner/Agent and submitted when required by HUD to disclose site-wide ethnicity reporting.  This form is a requirement with any Affirmative Fair Housing Marketing Plan (AFHMP) proposal, as well as any other funding request in which the Owner/Agent must report Race and Ethnicity.

This form is separate from the Race and Ethnic Data Reporting Form HUD-27061-H, which collects the individual data and is placed in the tenant file.
